As the onsite Health Coach, you will be responsible for working closely with the Physician Lead Care Navigator to coordinate personal wholistic health and wellbeing care team ensuring optimum experience and the opportunity for improved health and wellbeing outcomes. 

What you will do:

• Working together with Physician Lead Care Navigator to understand personalized health and wholistic wellbeing care plan
• Providing weekly reports of activities, progress, obstacles and next steps to Lead, Integrative Medicine Operations
• Providing onsite, daily personal coaching to support healthy lifestyle choices including nutrition, fitness, rest, and renewal
• Creating profiles of all care givers within Cross Care Management Platform
• Maintaining schedule: Coordinating and scheduling care team appointments, both in-person and virtual and coordinating schedule of all RG related meetings for Physician Lead Care Navigator
• Working with all care team members to ensure accountability for: Baseline assessment, care plan documentation in Cross electronic health record (EHR), including encounter/session with CEO, concern addressed and recommended next steps, document daily log of parameters that will be set by Physician Lead, outcomes tracking, and serving as scribe, as needed, for care team plan documentation
• Ordering and coordinating additional advanced predictive or diagnostic testing, as needed
• Participating in weekly Integrative Medicine Operations Meetings
• Work closely with Lead, Integrative Health Operations to update Dashboard weekly
• Traveling with individuals to encourage ongoing compliance to healthy lifestyle choices and components of personalized care plan
• Continue to update knowledge and best practices related to health and wellness
• Other duties as assigned

Who we’re looking for:

• A minimum of 5 years of experience in holistic clinical care or wellness services to include fitness and/or nutrition coaching
• Holds a current certification in this field
• Experience utilizing electronic health record (EHR) within a clinical care setting
• Superlative organizational and communication skills
• Ability to collaborative across multi-faceted teams of talent and personalities
• Superior attention to detail
• Highest level of professionalism augmented with unfaltering commitment to confidentiality
• Current passport and ability to travel nationally or internationally
• Ability to work outside of core business hours
