Coordinate high-volume patient workflows before and after in-home visits. Responsibilities include communicating lab results and follow-up instructions, validating documentation, managing clinical records, monitoring lab and imaging reports, coordinating with patients and healthcare organizations, maintaining provider information, protecting HIPAA-regulated data, and identifying process improvements.

Provides timely patient and partner support through phone, email, and SMS; triages and escalates concerns; coordinates real-time scheduling changes; supports field teams and appointment-related clerical work; tracks lab requisitions and results; maintains accurate records and patient confidentiality; and assists with process improvements and cross-training.

Conduct high-volume patient outreach, coordinate healthcare services, address social determinants of health, connect patients with community resources, and follow up through completion of care. Assess clinical and logistical barriers, educate patients, document interactions in compliance with HIPAA, collaborate with clinical and support teams, and escalate issues appropriately while maintaining an empathetic, high-quality patient experience.
