Provide nursing care in an Endoscopy GI Lab including patient assessment, individualized care planning, medication administration, peri-procedural monitoring, infection control, patient/family education, documentation, and emergency life-support. Perform in-home long-term care insurance assessments: collect claimant demographics, physician and medication information, conduct brief cognitive screening, evaluate ADL/IADL function, and document findings using a point-of-care web application on a laptop or tablet. Work is PRN and requires independent field visits.

Drive ACA Individual & Family Plan membership growth by recruiting, training, and managing independent agents/agencies across assigned Western states. Provide non-prescribing, in-home comprehensive assessments for Medicare Advantage and plan members: medication reconciliation, vitals, physical exam, evidence-based screenings, point-of-care testing, document diagnoses, communicate results to PCPs, identify urgent issues, educate members, address social determinants, and make referrals to improve health outcomes.

Provide advanced practice clinical care to high-risk older adults in nursing homes, assisted living, and senior housing. Perform comprehensive assessments, med reconciliation, point-of-care testing, documentation (ICD-10), advanced illness and care planning, patient/caregiver education, and collaborate with interdisciplinary teams to support short-term sub-acute rehabilitation.
