Coordinates care for Medicaid members and families receiving or recently discharged from NICU care. Conducts comprehensive assessments, develops person-centered care plans, coordinates providers and community resources, provides education and coaching, supports discharge transitions, and advocates for member needs. The role is remote within Washington and focuses on medical, behavioral, functional, cultural, and socioeconomic needs.

Provides comprehensive in-home assessments and clinical care for high-risk patients, including medication reconciliation, physical exams, screenings, diagnostics, acute and chronic condition management, rehabilitation support, documentation, care coordination, advanced care planning, and patient education. Travels throughout assigned states up to 85% of the time, Monday through Friday, as part of an interdisciplinary care team.

Serve as primary patient contact, schedule appointments/procedures, check in patients, verify insurance/referrals, document in EPIC, answer phone queues, manage film library/imaging records, cross-train and travel to cover other locations, and support team service and financial goals.
