The Manager, Connected Care Care Program for the Interdisciplinary team is responsible for providing direct supervision of the overall daily and long term operations and performance of a diverse team of healthcare professionals, including pharmacists, social workers, dietitians, and psychologists. You will drive team performance in delivering high-quality, patient-centered care and case management services, addressing the medical, psychosocial, nutritional, and mental health needs of patients in a virtual environment.
This role involves day to day operational oversight, resource allocation, and ensuring compliance with organizational policies and regulations., reporting directly to the Clinical Director, Care & Case Management and Utilization Management.
Job Description:
- Leadership and Oversight:
- Provide overall leadership and operational direction for the interdisciplinary team.
- Ensure implementation of strategic and process changes designed to achieve outlined goals/objectives.
- Oversee development of department specific goals and objectives, ensuring alignment with Care Management strategies and Included Health's vision, mission, and values.
- Monitor team performance and outcomes, ensuring alignment with OKRs and organizational goals.
- Operational Management:
- Oversee day-to-day operations of the team, ensuring efficient workflows and equitable distribution of caseloads.
- Quality Assurance and Compliance:
- Ensure that all team members adhere to professional standards, ethical guidelines, and regulatory requirements
- Monitor and evaluate the quality of performance by the team, implementing improvements as needed.
- Staff Development:
- Provide leadership, mentorship, coaching, and professional development opportunities for team members.
- Communication and Reporting:
- Serve as the primary point of contact between the interdisciplinary team and senior leadership.
- Prepare and present reports on team performance, patient outcomes, and operational metrics.
Requirements:
- Qualifications:
- Pharmacist
- At least 3 years of people management leadership experience in a healthcare setting.
- Excellent organizational skills and attention to detail required
- Strong interpersonal skills with ability to build relationships with employees
- Data-driven and comfort in using tools to evaluate and optimize the standardization of care across our growing team
- Ability to be agile and balance multiple priorities, while maintaining positive and professional attitude
- Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal
Physical/Cognitive Requirements:
- Prompt and regular attendance at assigned work location.
- Capability to remain seated in a stationary position for prolonged periods.
- Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment.
- No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer \/ laptop) may be required.
- Capability to work with leadership, employees, and members in an appropriate manner.
- Occasional overnight business travel.
