The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families.   

Responsibilities
  • All general responsibilities of a Staff Pharmacist. 
  • Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. 
  • Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions.
  •  Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution. 
  • Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management. 
  • Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.
Qualifications
  • Required:
    • Education:  Pharm. D. or BS in Pharmacy 
    • Experience: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
    • Licensure/Certification: 
      • Pharmacy license
      • BLS within 30 days of hire (ACLS and/or PALS if code attendance required);
      • Board Certification in Pharmacotherapy (BCPS)
