- Coordinates project plans, governance activities, meeting facilitation, action item tracking, communications, and reporting to support program objectives
- Leads operational readiness activities for new workflows, pilots, facility onboarding, and service expansions
- Partners with clinical, operational, analytics, and technology teams to gather requirements, prioritize work, and drive successful project execution
- Supports maintenance of operational workflows, process documentation, job aids, and supporting resources
- Manages system access and onboarding processes, including coordination of EMR access, security requests, and workforce readiness activities
- Identifies risks, develops mitigation plans, and escalates issues as appropriate
- Supports reporting and performance-monitoring activities to inform operational decision-making
- Builds and maintains effective relationships with internal and external stakeholders
- Leads or participates in cross-functional projects and continuous improvement initiatives

Required Qualifications:
- 3+ years of experience in healthcare operations, program management, project management, or clinical operations
- Experience managing multiple projects and priorities in a fast-paced environment
- Experience creating process documentation, workflows, and project plans
- Proficiency with Microsoft Office applications, including Excel, PowerPoint, Teams, and SharePoint
- Proven solid organizational, communication, and stakeholder management skills
Preferred Qualifications:
- Experience supporting care management, care coordination, population health, or value-based care programs
- Experience with operational reporting, workflow optimization, or process improvement initiatives
- Experience supporting program implementations, pilots, or operational deployments
- Knowledge of healthcare systems, hospital operations, or post-acute care workflows
