Provide clinical oversight of ECM Lead Care Managers (10–15 direct reports), supervise creation and execution of individualized care plans, escalate and manage complex/behavioral health cases, coach staff to meet KPIs, collaborate with PCPs and behavioral health providers, implement process improvements, and provide direct care as needed. Travel up to 20–25% for training and meetings.
